Medical Release Liability Medical History and Consent to Treat Waiver

Medical forms, the Waiver, and payments must be turned in one week prior to the start of the training session. Print the forms, complete them and submit with payment.

These forms only need to completed once from September 1st, 2007 until August 31st 2008,unless your contact or medical information changes.

Private Lessons

Private lessons are available by arrangement.

Most private lessons will include a substantial portion of dry land training, e.g., trampoline/dry board. A private lesson can include up to two divers, who may share the cost.

$150.00 - Twin City Diver Registration Fee
All divers are required to submit this prior to participating in club for the 2007-2008 season.
*When we receive your registration and the appropriate forms (Medical Release Liability, Medical History and Consent to Treat, and The Waver Form); we will register your diver with USA Diving and AAU.

Breakdown of the registration fee:
$75.00 USA Diving Registration - (valid October 2007 - December 31, 2008)
$16.00 AAU Registration - (valid September 1, 2007 - August 31, 2008)
$9.00 Administrative Fee
$50.00 Equipment Fee
New Lesson Format!

Please be aware when you register that we have changed the layout of our lesson groups. Please see the descriptions below.Please use your child's age as of December 31, 2008 in choosing your group.

High School Enrichment (HSE):
This group is designed for high school athletes wanting only one-meter experience, preparing for high school list. The cost is $150 per month ($450 for the season), and a $75 TCD Registration Fee. Please use the regular registration page to register for this group. HSE practice times are Sunday 11:30AM-1:00PM, and Thursday 6:00-7:30PM.

NOTE: The new costs of lessons are actually a decrease if you participate in all available sessions.The level of participation will be up to the individual.You must designate your training days and come on those days only.

US Diving Registration and AAU Membership
When we receive your registration and all of the appropriate forms (Medical Release Liability, Medical History and Consent to Treat, and The Waver Form), we will register your diver with USA Diving and AAU.

* US Diving Registration: REQUIRED for all Team Members (2008 membership now available, good thru December 31, 2008)
US Diving Registration is now available on-line. Go to www.usadiving.org

** AAU Membership: Required for all Twin City Diving Members (Valid September 1, 2007 - August 31, 2008)
If instructed by Twin City Divers, you can register and pay online at www.aausports.org. Our club code is MNDI5TBKB7. Please register as a member with added benefits the cost is $16.00.

*** Annual Club Membership: Valid September 1, 2007 - August 31, 2008.
Make club membership fee checks payable to 'Twin City Divers'. This is an annual fee and applies to all members.

Casual Basis/Walk-in
This option has been phased out.


Policies
  • Payment is due one week before the first day of each training session. Divers who have not paid will not be allowed into the facility. A $10 late fee will apply.
  • All TCD participants must have proof of AAU membership for the current year. (Contact Jim for registration information. See Contact TCD.)
  • Make-up sessions are not permitted, unless you have a note from a physician or training has been cancelled.
  • There are no refunds, unless a class is cancelled.
  • If a class does not fill up, we reserve the right to cancel or combine classes. If we cancel a class you will receive a full refund.
  • A 10% discount applies to the second child in the family, 15% discount for third child.
  • The second child from a family pays half the regular TCD annual registration fee. The third child pays no annual registration fee.
